To request a withdrawal from class, please fill out the form below.  Withdrawals will take effect on the 1st of the month.  No refunds will be issued for tuition already paid, costumes, or competition fees.  New students withdrawing within the 30-day time frame will be issued a refund of their registration fee.  To transfer to a different class, please do not fill out this form.  Transfers can be completed by contacting the office.  Students who withdraw and wish to return later will be required to pay the regular $25 registration fee.
